Pain, swelling and stiffness at the base of the big toe, especially when pushing off or bending the toe upward, is the hallmark of turf toe — a sprain of the ligaments and joint capsule supporting the big toe’s main joint (the first MTP joint). It’s common in sports involving sprinting, jumping and sudden acceleration, particularly on firm or artificial surfaces, and ranges from a mild strain to a more significant ligament tear.
“Turf toe” is the common name for a sprain of the first metatarsophalangeal (MTP) joint — the big toe’s main knuckle joint — caused by the toe being forced upward beyond its normal range, usually while the foot is planted and under load.
It typically happens when the big toe is forced into hyperextension — for example, the front of the foot fixed on the ground while the heel lifts and body weight drives forward, common when accelerating, jumping, or being tackled from behind in sports like football and rugby. Firmer playing surfaces and more flexible footwear increase the risk, which is part of why it became more common with artificial turf.

Severity ranges from a mild ligament sprain that settles in a couple of weeks to a more significant tear of the joint capsule and ligaments that can take considerably longer. A physio assessment early on helps determine which end of that spectrum you’re dealing with and will flag the small number of cases — ongoing instability or pain that isn’t settling with initial management — that warrant a further opinion.
Initial treatment focuses on protecting the joint from further forced bending — sometimes with stiffer-soled footwear or a supportive taping technique — alongside pain-guided movement. Physio rehab then restores joint range of motion and strength and works on the movement patterns (like push-off mechanics) that load the toe, before a graded return to sport.
Sprinting, jumping, forceful push-off, and flexible-soled footwear or artificial turf all tend to aggravate turf toe. A stiffer-soled shoe or taping to limit how far the toe bends upward, avoiding push-off-heavy activity while it heals, and gentle guided range-of-motion work tend to help.
Mild sprains often settle within two to three weeks; more significant injuries can take six to eight weeks or longer. Returning to sprinting and cutting sports before the joint has regained full strength and range is the main reason turf toe becomes a recurring niggle, so a staged return matters here too.

Core Healthcare is looking for an experienced Health Psychologist to join our multidisciplinary Allied Health team in Brisbane. You’ll lead Adjustment to Injury Counselling (ATIC) and Psychological Return to Work (PRT) as part of a structured Chronic Pain Management Program — supporting patients navigating work-related injuries, motor vehicle accidents, or DVA-funded claims.
This isn’t a solo practice. You’ll be embedded in a genuine multidisciplinary team, working shoulder-to-shoulder with chronic pain physiotherapists, exercise physiologists, pain medicine specialists, and return-to-work rehabilitation providers to deliver holistic, evidence-based care.
Your focus will be helping patients adapt — mentally and emotionally — to persistent pain, so they can rebuild function and return to meaningful work and life.
Day to day, you’ll be:
✓Delivering ATIC therapy within a structured Pain Management Program
✓Conducting Psychological Return to Work (PRT) assessments and interventions for injured workers on worker’s compensation
✓Collaborating on case reviews, treatment planning, and outcome monitoring with your MDT colleagues
✓Providing a blend of face-to-face and telehealth consultations to an established patient caseload
✓Producing clear, timely clinical documentation — including Provider Management Plans and insurer reports — in line with worker’s compensation requirements

You’ll bring:
• Current AHPRA registration and valid professional indemnity insurance (Health Psychology endorsement preferred)
• At least 5 years’ experience as a psychologist
• Demonstrated experience in ATIC, particularly with chronic and complex pain presentations
• A solid understanding of the worker’s compensation system and multidisciplinary pain programs
• Interest in PRT — including functional capacity, psychosocial barriers, and goal-oriented recovery planning
• The right to live and work in Queensland
Bonus points if you’re trained in EMDR — it’s not essential, but it’s highly valued here.
